Bring with you all initial paperwork, printed and completely filled out, and any diagnostic reports such as x-rays, MRI’s, and lab tests as it pertains to your condition. • Do you accept my insurance?
Yes, we accept all insurance! If we are not an “in network provider” for your specific carrier we will be able to provide you with a reimbursable super bill. We bill your insurance as a courtest to you. You have a contract with your insurance carrier and ultimately you will be responsible for any services that are not covered under your policy. We will pre-authorize you for any treatments and you will be informed if the services rendered are covered.
